Resumption of Service
If your service was discontinued for non-pay you will have to fill out the PDF form below and email it to us along with a copy of your driver's license.
To get your service reestablished, please fill out the form along with a copy of your driver's license and email it to atyourservice@ubon.us. Doing this on iPhone will require you to save the file to your device first, then opening it from your files.
You can also text a picture of your driver's license to (205) 446-5757 if you have difficulties emailing it.
Alternatively, you can visit our office in person to have your service resumed.
